MYHockey Tournaments Registration Form

Upon receipt of your registration, MYHockey Tournaments will hold a spot for your team for 5 days or until a deposit of $250 is received. All balances must be paid in full 45 days prior to the event. All teams will be required to submit a certified roster from USA Hockey or Hockey Canada prior to the tournament start date. Teams are not required to have their certified roster at the time of entry. Please mail your payment to:
